Thank you for your interest in the CBH CO-OP Foundation. We are proud to support organizations and initiatives that align with our mission of strengthening rural communities, supporting youth in agriculture, and enhancing local vitality.

This form is intended for individuals, nonprofits, schools, and community groups requesting financial support or in-kind donations from the CBH CO-OP Foundation. Please provide as much detail as possible so our board can thoroughly review your request.

If you are requesting event sponsorship (support that includes logo placement, advertising, or public recognition), please complete the CBH CO-OP Sponsorship Request Form at http://www.cbhcoop.com/forms.

For student scholarships, please watch for our annual CBH CO-OP Foundation scholarship application process, usually announced each spring.

Requests are reviewed regularly, and applicants will be notified by email once a decision has been made.

Donation Request Terms & Conditions

By submitting this request, the applicant acknowledges and agrees to the following terms:

  1. Submission Does Not Guarantee Approval  Submission of a donation request does not guarantee funding. All requests are reviewed and approved at the discretion of the CBH CO-OP Foundation.
  2. Review Timeline  Requests must be submitted at least 30 days prior to the event or funding deadline to be considered.
  3. Use of Funds  If approved, donated funds must be used solely for the purpose described in the application. Any changes to the intended use must be disclosed and may require reapproval.
  4. Proof of Nonprofit/Organization Status  The Foundation may request supporting documentation, including proof of nonprofit status, tax ID number, event details, or financial information.
  5. Promotion & Recognition  The applicant agrees that the CBH CO-OP Foundation may use the organization’s name, logo, and event information for recognition and promotional purposes (website, social media, newsletters, etc.).
  6. Reporting Requirement  Approved recipients may be asked to provide a follow-up summary, receipt, photo, or documentation showing how the donation was used.
  7. Right to Decline  The CBH CO-OP Foundation reserves the right to decline any request that does not align with its mission, values, or funding priorities.
